What documentation is accepted?

Accepted documents include:
- Veteran designation on a driver’s license
- Veteran Identification Card
- Current paystub for active-duty military personnel
Educators and medical personnel
Accepted documents include:
- Employee or volunteer badge
- Paystub issued within the past 60 days
- 1099 tax form from the previous year
First responders
Accepted documents include:
- Employee or volunteer badge
- Paystub issued within the past 60 days
- 1099 tax form from the previous year
- Signed affidavit issued by the first-responder agency
How do I place a Community Cares order?
Follow these steps:
- Download and complete the Human Touch Community Cares Order Form.
- Include your order information and at least one accepted eligibility document.
- Submit the completed form and documentation according to the instructions on the form.
- Allow approximately one to two business days for Human Touch to review the documentation.
- After eligibility and order details are confirmed, Human Touch will provide an order confirmation and process payment.
No charge will be placed on your credit card until Human Touch has confirmed the order details with you.
